I have often errors index $I30, if defragment MFT or(and) files.
STEALTH & SPACE
And I have every time the CHKDSK repair indexes.
Why is this happening?
In comparison with all other competitive defrag tools for Windows I do not have these problems.
It is not the fault of the drive, because I have a lot of drive and on each the same !!!
Index error occurs most often on huge drives with lots of small files.

any defragmentation does not damage your disk - unless the disk is already damaged. We only use the Windows-API, therefore any defragmentation run will always be safe.

If you have issues with the filesystem, use CHKDSK to fix them.
